Robert Cohen

President & CEO

Mr. Cohen occupied senior management positions at Pfizer Inc., Sulzermedica, and St. Jude Medical. He most recently founded Miromatrix Medical Inc. (fully biologic human organs) and grew that enterprise to a $90-plus million valuation over seven years by technology development, creating and commercializing two products, extensive partnering and business development activities, regulatory and reimbursement success, and $31 million in fund raising. Immediately prior, Mr. Cohen built significant shareholder value as CEO of Travanti Pharma Inc. (iontophoretic drug delivery) through technology, product and business development, followed by the successful sale of the company to Teikoku Pharma. Diverse activities have included successfully taking a company public on the NYSE, initiating and developing new products through to market, establishing a manufacturing facility in a foreign tax-friendly environment, a series of acquisitions, divestitures and licensing transactions ranging from several thousand dollars to almost one billion dollars, raising capital for early stage companies and building sales organization in the U.S. and abroad. Mr. Cohen serves or served on the Board of Directors of seven medical technology companies. He is able to leverage thirty-five years of investor, clinical, distribution, executive, technology, venture capital and investment banking relationships.

Jafar Golzarian, MD

Chief Medical Officer

Dr. Golzarian is the founder and Chief Medical Officer of EmboMedics and inventor of the Company’s technology. He is an Interventional Radiologist, certified by the American Board of Radiology, and an internationally recognized expert and opinion leader in the field of embolization. Dr. Golzarian is a pioneer in the field of embolization, having performed multiple first-time procedures using microsphere technology in the United States for several indications. He is the author of more than 60 peer reviewed articles. Dr. Golzarian has served as Professor of Radiology and Surgery, and Division Head, Interventional Radiology and Vascular Imaging, at the University of Minnesota since 2008. From 2003 to 2008, he was a Professor of Radiology at The University of Iowa Hospitals and Clinics, and from 1994 to 2003 he was Chef de Clinic Adjoint at Erasme Hospital, University of Brussels. He graduated from the University of Brussels School of Medicine, Brussels, Belgium in 1988 and, from 1988 to 1994, held a Residency in Radiology and a Fellowship in Interventional Radiology at Erasme Hospital, University of Brussels.

Jon Freeman

contract Chief Financial Officer

Mr. Freeman has 25 years of finance, operations and management experience across a variety of industries including medical device, technology, industrial manufacturing, and professional services. Most recently, Mr. Freeman was Chief Financial Officer of Medafor, Inc. (Minneapolis, MN). Also he has worked in environments ranging from large public multinationals to smaller, emerging private companies. Mr. Freeman has served in management roles in several companies which have created and realized substantial shareholder value and also has extensive experience with a number of high-performance management systems. He began his career in Management Consulting at KPMG. Mr. Freeman has a Master’s in Business Administration from the University of Texas and a Bachelor of Arts in Economics from St. Olaf College, as well as receiving the Certified Financial Management (CFM) designation from theInstitute of Management Accountants

Gerald D. Van Eeckhout

Mr. Van Eeckhout joined the Company’s Board of Directors as Chairman of the Board in 2014. He is presently Chairman of Evergreen Enterprises LLC, a private consulting company. He most recently served as CEO of ConferTech International (NASDAQ) for eight years and CEO of ACT Teleconferencing (NASDAQ) for thirteen years. His management career includes seven years as Controller, Vice President, Chief Financial and Administrative Officer of Medtronic Inc. (NYSE), five years as Chief Financial and Planning Officer of The Pillsbury Co. International Division, and eight years as a CPA with Touche Ross and Co., all in Minneapolis, Minnesota. Mr. Van Eeckhout served on the Board of Directors of Medafor, Inc. (sold to BARD in 2013) and served as Chairman of Medafor’s audit committee. He received a BS in Accounting from the University of North Dakota and is a graduate of the Stanford Business School Executive Program. He has served as an outside director of several public and private companies in the US and China in the medical technology, telecommunications, food, and non-profit sectors. He is a former National Director of the American Electronics Association, a past President of the University of North Dakota Foundation, a current Trustee of the University of North Dakota Entrepreneurial Studies Foundation, a former board member of the Rocky Mountain World Trade Association and a former chairman of the Chamber of Americas. He was named the 2001 Ernst and Young “Entrepreneur of the Year” for the Rocky Mountain region. He has served as a Professor of Accounting, Finance, Marketing, and International Management.

Reza Malek, MD

Dr. Malek is an Interventional Radiologist, boarded in Radiology and Interventional Radiology, and is a full member of the Society of Interventional Radiology and the Society of Neurointerventional Surgery. He was the first physician to bring endovascular neurosurgery to San Jose and developed one of the most active stroke programs in the country. Dr. Malek specializes in all aspects of peripheral vascular disease and endovascular neurosurgery and is active in teaching, research, and publishing in this field. Dr. Malek completed medical school at Jefferson Medical College of Thomas Jefferson University in 1991 and continued his internship and a diagnostic radiology residency at the Medical Center of Delaware, a Thomas Jefferson University affiliate. He completed a cardiovascular and interventional radiology fellowship at Brigham and Women’s Hospital/Harvard Medical School in 1997. Dr. Malek was the chief of Interventional Radiology at Riverside Community Hospital and St. Mary’s Regional Medical Center for five years. He also was an assistant clinical professor at University of California Irvine Medical Center. In 2001, Dr. Malek was acting Chief of Interventional Radiology at the University of California, Irvine Medical Center and continued as a part-time fellow in Neurointerventional Radiology. In 2003 Dr. Malek completed another year of fellowship in endovascular neurosurgery at University of California, San Francisco. Dr. Malek recently has finished a Master’s Degree program in Neurovascular Diseases from SUD University of Paris under one of pioneers of the field – Professor Pierre Lasjunias.

Robert Halverson

Mr. Halverson has served as a Member of the Board of Directors of the Company since early 2015. He recently served seven years as a Member of the Board of Directors of Medafor, Inc. a medical technology company based in Minneapolis, MN which, in October of 2013, was acquired by C. R. Bard (BCR – NYSE). Mr. Halverson retired from Northwest Airlines (NWA) in 1998 as a pilot with more than 32 years of service and at retirement was flying Captain on a Boeing 747 based in Anchorage Alaska. Although retired as owner/operator from active grain farming begun in 1975, he and his wife continue to manage the family farm land in Western Minnesota. Mr. Halverson currently is involved as an early round investor in two additional medical startups. He is an authorized volunteer mentor for the Alaska Department of Corrections and works closely as an advisor with Mission Aviation in Alaska. He has served in the United States Army and Ready Reserve as a Platoon Sergeant and later volunteered as a Northwest Airlines pilot attached to the CRAF (Civil Reserve Air Fleet) operation during the first Persian Gulf War in 1991, flying troops and equipment among the USA, Frankfurt, Germany and King Fahd and Dhahran Airports in Saudi Arabia. Mr. Halverson also served on the Minnesota, City of Minnetrista, Park and Planning Commissions and as a member of the Anchorage Alaska Civil Air Patrol. He was elected in a statewide election and served three years as a Director of the Minnesota Corn Growers Association, was formerly a Secondary Mathematics teacher in Forest Lake MN and Bloomington MN public schools, and received his Bachelor of Science degree in Secondary Education from St Cloud State University with a major in Mathematics and a minor in physics and chemistry.

Scientific Advisory Board

Marc R. Sapoval, MD, PhD

Dr. Sapova is a professor of clinical radiology and chair of the cardiovascular radiology department at Hôpital Européen Georges-Pompidou in Paris, France. He is a Member of RSNA, SCVIR, SFR, ESR, SFAV, Fellow of CIRSE and was the Program Chairman for CIRSE 2008-2009. Dr. Sapoval is an expert in the area of renal disease and will assist EmboMedics in area of embolizaiton for renal disorders.

Mahmood Razavi, MD

Dr. Razavi specializes in image-guided therapy for cancer and endovascular treatment of vascular diseases. Before becoming the Director of Clinical Trials and the Research Center at St. Joseph Heart & Vascular Center, he held faculty positions at UCLA and Stanford University. Dr. Razavi chairs the Interventions Committee for the ATTRACT Trial, determing the best treatment for patients with proximal DVT. He obtained an MD from the University of Southern California and undertook his residency and fellowship at UCLA and Stanford. He is the Editor-in-Chief – Techniques in Vascular & Interventional Radiology, and is a member of Council on Cardiovascular Radiology and Intervention – American Heart Association. Dr. Razavi chairs the Scientific Program – Society of Interventional Radiology.

James B. Spies, MD, MPH

Dr. Spies is the Current President of the Society of Interventional Radiology and the Chair and Chief of Service at the Department of Radiology ,Georgetown University Hospital. He is one of the most experienced researchers in the field of uterine artery embolization. He has been an active researcher in the field since 1997 and has over 45 peer-reviewed publications of studies from this procedure. His group has defined a number of parameters associated with uterine embolization, including ovarian function after fibroid infarction as a marker of success, relative success of embolic materials, hypercoagulability after treatment and the long-term outcome after treatment. He has been invited to present over 250 lectures on uterine fibroid embolization and is recognized as a leader in the field. He obtained his MD from Georgetown University School of Medicine and his MPH from Johns Hopkins School of Public Health.
